Offers health care to the Lower Sioux Indian Community.
The Community Health Services Program includes:
* Contract Health Services - coordinating with outside health services
* Community Health Nursing
* Health Promotion
* Diabetes Coordinator
* Dietician Services
Services include:
* Home monitoring of vital signs
* Wound Care
* Diabetes Care
* Individual plans of care for specific needs
* Monthly health education meetings
* Blood pressure and diabetes screenings are available during regular business hours in their office
* Help members with insurance questions
* Flu immunizations (shots) annually
* Annual Health Fair
* Health management
* Health education

Eligibility
Service is provided in the counties of Redwood and Renville within a 10-mile radius. For some services, must be eligible for Indian Health Service (IHS) service.
