Description

Provides supportive services to vulnerable adults of the community and their caregivers


Services provide includes:
* Telephone consultation, information and assistance, and referrals
* Assessment of needs and situation with recommendations for the least restrictive means to meet needs
* Advocacy and coaching
* Petition the court and provide testimony for persons in need of guardianship or conservatorship
* Guardianship and conservatorship
* Evaluating for less restrictive alternatives
* Supported decision making
* Training, support and evaluation of professional guardians or conservators


Eligibility

Older adults, families and caregivers in the seven-county metro area or statewide.
